چکیده

Pain is common after a bone graft has been taken from the iliac crest (Kurz, Garfin and Booth 1989). Local wound infiltration with bupivacaine at closure is effective, but relief lasts for only four hours (Todd and Reed 1991). We report a method of infusing bupivacaine which gives effective and lasting analgesia. Patients and methods. Patients requiring iliac crest grafts were randomly selected to receive either bupivacaine infiltration at wound closure or bupivacaine infusion postoperatively. For the infusion group, a fine-bore catheter was tunnelled into the wound between muscle and fat and used to infiltrate 0.5% bupivacaine solution at a rate of 5 ml/hour for 48 hours by a syringe driver. A drain was also used; drain and catheter were removed after two days. For the infiltration group 10 ml of 0.5% bupivacaine solution was injected into the soft tissues by needle and syringe immediately before skin closure. A drain was used. At 24 hours postoperatively, patients graded their

Evaluation of the efficacy of bupivacaine soaked in Gelfoam(®) at the iliac crest bone graft site.

BACKGROUND AND OBJECTIVE A substantial number of patients report pain after graft harvest from the anterior iliac crest. This study examined the efficacy of local application of bupivacaine soaked in a Gelfoam(®) at the bone donor site in controlling postoperative pain and parenteral opioid use. METHOD We performed a prospective, double-blind, randomized, placebo-controlled study comparing in...

Comparative Study of Proximal Tibia and Iliac Crest Bone Graft Donor Sites in Treatment of Orthopaedic Pathologies

Introduction: Bone graft is harvested from bone and used to stimulate bone healing due to its properties. The aims of the study were to compare the quantity of graft harvested from proximal tibia and iliac crest, and the complications at these donor sites. Materials and Methods: This was a prospective study carried out on all consecutive patients who had orthopaedic procedures that required bon...

Autogenous bone harvesting and grafting in advanced jaw resorption: morbidity, resorption and implant survival.

AIM To analyse the morbidity arising from autogenous bone graft harvesting, graft resorption and implant survival in grafted sites. MATERIALS AND METHODS Only comparative clinical trials on the harvest of autogenous bone grafts were selected. Studies were excluded if they compared autogenous bone grafts to bone substitutes or vascularised free bone grafts. RESULTS A total of 24 studies were...

Prospective observational study of donor-site morbidity following anterior iliac crest bone-grafting in orthopaedic trauma reconstruction patients.

BACKGROUND Complications associated with iliac crest bone-graft donor sites have been reported. This prospective study was conducted to determine the prevalence of pain and complications at the iliac crest donor site in patients undergoing treatment of fracture nonunion. METHODS Ninety-two patients undergoing anterior iliac crest bone-grafting for nonunion or delayed union of a long-bone frac...

Complications following autologous bone graft harvesting from the iliac crest and using the RIA: a systematic review.

Bone grafting is a commonly performed surgical procedure to augment bone regeneration in a variety of cases in orthopaedic and maxillofacial surgery. Autologous bone graft remains to be the 'gold standard' and the iliac crest to be the most common harvesting site. The intramedullary canal of long bones represents another potential site for large volume of autologous bone graft harvesting and is...
